ATHLETIC FORMS

Before participating in any school sport, students must have a current physical form on file. Please download and complete the forms below and submit all items to Dr. Manno.

Overhills High School athletics will only accept the physical form from the NCHSAA. If you have any questions, please contact the athletic director, Dr. Manno. Thank you for your interest in Overhills athletics.
